Patient Navigator Resource Guide

Patient navigators help patients overcome barriers to healthcare.  Roles for patient navigators vary depending on the institutional setting.  Professional requirements for navigators also vary, with some navigator programs drawing upon lay health/community health educators and other programs utilizing health and social service professionals.  Evidence of the efficacy of patient navigators in impacting health outcomes, especially in medically underserved populations, is growing.

The American Public Health Association offered several sessions related to patient navigation at a recent conference.  Materials and links can be found by searching for 'patient navigation.'

Patient Navigator Program Overview- The Institute for Alternative Futures published a 2007 overview of patient navigation programs.
